Evidence Based Mental Health Nursing Introduction This report will focus on the assessment and care planning for an individual using the mental health services. Evidence based approach will be deploy in order to be able to record, review and monitor the progress of the service user. Evidence-based practice is a structured and systematic approach to using research based knowledge of effectiveness to inform practice (Olfson, 2009). Knowledge includes formal information derived from research, for example from published trials and reviews. It also encompasses the informal knowledge and wisdom of practitioners, sometimes called tacit knowledge. This informal knowledge can include, in addition, the expertise of those who receive an intervention, whether that is medication, talking therapies or attending a parenting skills group. Care Programme Approach(CPA)provides the framework for assessing and care planning for a person with mental illness. CPA was introduced in 1991 by Department of Health as a systematic arrangement for assessing the health and social needs of people accepted into specialist mental health services (Kingdon, 1998). The formation of the plan will detail the health and social care required from a variety of provider as well as the appointment of a key worker to keep in close touch with the service user and to monitor and co-ordinate care. In addition, regular reviews will be conducted where necessary, agreed changes to the care plan. Hence, in this report, I will outline the main principles of CPA and deploy the use of Tidal Model and principles throughout the report by Tidal models principles to clinical practice and assessment and care planning. A detailed background and information about the individual receiving care in the clinical section of where I work will be provided. And to make it clearer for the audience of this report, I will highlight the purpose of conducting this assessment as well as the process of the assessment. The discussion will centre on care planning and the strategies or issues that may impact the process. In order to keep the identity of the person under care anonymous in in accordance with confidentiality and the NMC code, only the initial of the individual will be used through the discussion in this report. As a nurse, I owe a duty of confidentiality to all those who are receiving care under me and that includes making sure that they are informed about their care and that information about them is shared appropriately (Maloney, 2016). Care Programme Approach Framework and The Tidal Model The Care Programme Approach (CPA) is a way that services are assessed, planned, co-ordinated and reviewed for someone with mental health problems or a range of related complex needs (Nhs.uk, 2017). CPA are generally offered to individual that have been diagnosed for having a severe mental disorder, someone who is at risk of suicide, self-harm, or harm to others and people with history of violence or self-harm. In addition, the service is available for people that vulnerable, this could be for different reasons such as physical or emotional abuse, financial difficulties because of mental illness or cognitive impairment. The three main core principles of CPA according to Rowland (2013) are the assessments of the needs of the individual, allocation of a care coordinator and plan how to meet the needs of the person. The coordinator will ensure that the plan include the fully assess of the service user needs, it will also show how the NHS and other organisations will meet the needs of the person, including the family in some cases. It has to be regularly reviewed by the coordinator to monitor progress. In addition, the coordinator will have to think about all the mental health needs of the service user, medication and side effects, employment, training or education and personal circumstances including family and carers. The assessment will include the risk of the service user to themselves or other, either there is a problem with drugs or alcohol. The CPA is a model for good practice which remains applicable today. However, the CPA is a care for those of working age in contact with specialist mental health and social care services (Donohue, 2014). It is crucial to work have an integrated approach across health and social care to minimise the distress and confusion sometimes experienced by people referred to the mental health system and their carers. In addition, professionals have found some aspects of the CPA over-bureaucratic, managers and service users alike have found the lack of consistency confusing (Donohue, 2014). It is they who have been working and living with the CPA for some years now and it is important to take account of their views. In the nutrshell, Bree-Aslan and Hampton (2009) indicated that CPA is not a model of care but a tool and process to guide nurses on how to provide effective service for people with mental illness. By embracing an integrated approach where by a seamless service can be achieved through an integrated approach to care co-ordination which provides for a si ngle point of referral and a unified health and social care assessment process (Koopmans, 2013). Tidal model will be deployed through the care planning and assessment in this report. This principles and philosophies of this model will help to give an in depth understanding to the process of assessment and care-planning. Tidal model is a mental health recovery model which may be used as the basis for interdisciplinary mental health care. It was developed by Dr Phil Barker and Poppy Buchanan-Barker as a philosophical approach to the discovery of mental health (Barker and Buchanan-Barker, 2010). The Tidal Model accentuates helping people reclaim the personal story of mental distress, by recovering their voice. With service user, own language, metaphors and personal stories, people can begin to reclaim the meaning of their personal experiences. Helping someone to a problem in living is rarely easy because everyone is unique and each persons reaction to any problem in living also is unique. Hence, what works one person may not always work for another. In Tidal model. The first step towards someone with mental illness recovering control over their lives. The model enable mental health nursing to be used as the basis for interdisciplinary mental health care and the focus begin with begin with the recovery journey when the person is at their lowest ebb experiencing the most serious problems in living (Barker and Buchanan-Barker, 2010). The Tidal Model provides a practice framework for the exploration of the patients need for nursing and the provision of individually tailored care. (Barker P, 2001) and it is considered as a mid-range theory of nursing, hence the main focus of the model is on helping individual people, make their own voyage of discovery. From the research, already been conducted by different scholars, the combination of CPA framework and Tidal model with the collaboration with the service user will enable them to recognise areas and needs that will be most suitable for their recovery as well as promoting a culture of p erson-centred care that is not associated with CPA framework. Biography Janet Bonet is a 58-year-old female living with her daughter who is one of her three children. Janet has never been married and also has no partner at the moment, although she said to have been in different relationships in the past but which seem to lead nowhere. My mentor and I have been asked to assess Janet during her inpatient appointment at the centre. Janet has been known to mental health services over the years and has a history of disengagement. She has had a diagnosis of severe depression in the past and also suffers from back problem which impacts on her mental health. Janet has never been able to keep a job due to her physical health and so she has been in benefits most of her life. Due to financial stress in the past, she has self-neglected and now depend on her carer who is also her daughter for support in daily activities. Also, she was asked by the council to downsize her four-bedroom house to a two-bedroom house which she has done but still waiting on the housing list to be moved closer to her family and she finds this waiting period stressful as she has been waiting for over a year so housing is also her concern. Recently, she has been experiencing a lot of fluctuations in her mood as she reports that she lost her Dad, Mum,Nan,and Grandad within two weeks of each other and found this extremely distressing and also has been having thoughts of harming herself. Her daughter is her carer and she also claims that her other children along with her grandchildren do visit sometimes which makes her think less about self-harming herself. She says My children and grandchildren are my protective factors. She went further to say that she has been experiencing feelings of emptiness and anger due to the fact that she does not feel safe in her neighbourhood as she feels some people are out to get her. Janet admits that in the past she has not been compliant with her medication due to side effects but is willing to be compliant with treatment now that she feels she is in crisis in order to promote her recovery. Janet has also reported suicidal thoughts in the past and has had two attempts at committing suicide. Assessment The assessment of service user with mental illness include collections of different range of information. The information may include mental health symptoms and experiences of the service user, feelings, thoughts and actions physical health and wellbeing, culture and ethnic background, use of drugs or alcohol, social and family relationships and past experiences, especially of similar problems. The whole essence of conducting assessment is for the coordinator to be familiar with the life history of the service user. However, I prefer deploy Barker (2008, p.66) procedures of assessment by trying to answer the question who the service user is. The procedure will enable the coordinator to focus on the individual as a whole by considering their needs instead on focusing on diagnosis. The second question like what is wrong? will prompt the service user to give information about their state of mind and wellbeing. For a nurse, the concept is to show empathy for the service user to be at eas e and feel unthreatened to give more information about the state of the health and their experience. The correlate with Tidal model of Barker and Buchanan-Barker (2010) that indicated that coordinator can deploy a holistic assessment whereby the service user is allowed to tell their story and world of experience. Through holistic assessment, therapeutic communication, and the ongoing collection of objective and subjective data, nurses are able to provide improved person-centred care to patients. A holistic assessment approach acknowledges and addresses the physiological, psychological, sociological, developmental, spiritual and cultural needs of the patient (Kreys, 2014). However, finding out who the person is and what is wrong with them, is just one part of the assessment. It is essential to form a collaborative and empowering relationship with the service user in order for them to be comfortable around the coordinator. The Inequality Between the Low and High Class in Brazil
Brazil is the largest country in South America and it has the strongest economy in Latin America. The country has the seventh largest economy in the world by nominal GDP. Brazil is rich in natural resources and it focus on agriculture and industrial power. Despite the improvement on income distribution and bringing the middle class population to 95 million people which is a little bit more than half of the population in the country, poverty in rural areas are still very severe in Brazil. According to Rural Poverty Portal, ââ¬Å"In the country as a whole, about 35 percent of the population lives in poverty, on less than two dollars a day. (1) The population in Brazil is about 197 million and with 35 percent of population living in poverty is equal to two times the population in Canada. Most of the poverties in Brazil are concentrated in the North East region of Brazil and it can be considered the single largest concentration of rural poverty in South America. The North East region in Brazil is the undeveloped part of the country where the population have no access to education, health care, technology and even clean water. Several causes of poverty in Brazil are land tenure, lack of access to a good education and also skill training. Through the literary short stories and Brazilian made films, we can somehow picture how different is the life between the lower and higher class families. The inequality is a very big issue in the country and even though the stories and films are fiction, it still shows us a reality that Brazil has been facing for a long period, which is the big gap between the rich and the poor. One of the films that shows the poverty in the rural areas of Brazil is ââ¬Å"Behind the Sunâ⬠, directed by Walter Salles. We can see how the violence is a very big problem in the story because of a land dispute between a family and neighbour. According to Scielo, ââ¬Å"It is customary to state that the poorer strata of the population are more violent and that they cause the social disorder and disturbances that assail the country. â⬠( Minayo, 1) It is true that the lower class would be more involved in violence since they are tired of working so hard and not getting anywhere, and it affects them emotionally. Furthermore, the film starts with a background of very dark orange which may represent the very hot weather in Brazil and it makes the viewers to somehow feel the tiredness, the hard working people in rural areas in Brazil. The film begins with a very depressing color which almost represents ââ¬Å"hopelessnessâ⬠. The scene that most caught my attention and I think it was very meaningful to the viewers is when the oxes start to turning in circle over and over again. It brings a significant message of no matter how hard the people work in rural areas; they will always be in the same routine and will never be able to have the chance to succeed in life. Moreover, as we could see, one of the main characters, Pacu, a very simple and humble boy, speaks a Portuguese with several grammatical errors such as ââ¬Å"mais melhorâ⬠and ââ¬Å"eles que tava perdidoâ⬠. Illiteracy is another big problem in Brazil, especially with the lower class population, who live in the rural areas. Lower class people that live in the interior do not value education as much as people from higher classes. Sometimes it is because they are so focused on working to support their family that they do not have time to think about educating their children. Other times, they do not even have money to buy food, so how are they going to support their children to go school? Many children who are born in lower class families do not have the opportunity to be educated, not because they do not want; however, it is because their parents cannot afford and they are incapable of sending them to school. There are approximately 14 million people that are illiterate in Brazil; which means that they cannot read and write. Fortunately, in 2003, Brazil launched the ââ¬Å"Bolsa Familiaâ⬠, where the government support poor families with 140 Reais which is equivalent to 80 dollars to benefit the family to send their children to school, accessing health care and other social assistances. In addition, the literary story ââ¬Å"The Hour of the Starâ⬠by Clarice Lispector also helps the reader to imagine how the lower class women's lives seem like. In recent years, women had played a big role in the contribution to the Brazilian economy. Today, Brazil even has a woman president in charge of the country, Dilma Rousseff. Wealthy women in Brazil seem to always be able to have a free pass to succeed in life, although many lower class women in Brazil do not receive the same opportunity. Higher class women always succeed thorough high society connection or using the power of the money. Macabea, the main character, from ââ¬Å"The Hour of the Starâ⬠can represent all the lower class women in Brazil. Since Macabea lost her parents when she was very young, she became an orphan. Therefore, that is probably one of the reasons why she had difficulties succeeding in life due to the absence of parents supporting and guiding her. According to SOS Childrenââ¬â¢s Village, ââ¬Å"In regions that are marked by high unemployment rates, children often face a substantial risk of growing up in an unstable domestic environment. ââ¬Å"(1) It is possible to imagine how an orphan girl would need to go through without parentââ¬â¢s guidance. Moreover, the story also highlights that due to Macabea status of being poor, her lover, Olimpio, believes that he would not have any chance of advancing in life being with her. Instead, he chooses to date her co-worker, Gloria, who was smarter and prettier than Macabea. In addition, the title of the narrative, ââ¬Å"The Hour of the Starâ⬠, is about Macabea who wanted to be a cinema actress. Unfortunately, people would not pay attention on her and the fact of her being poor, there was almost no chance for her to become who she wanted to become, which is a very sad reality that actually exists in Brazil. In Brazil, the poor still suffers inequality, they have almost no chance to succeed in life and they are pretty much ignored by the society. The poor is almost invisible to the society and the poor keeps getting poorer. For Macabea, she only sort of becomes famous after being hit by a Mercedes Benz, which is a ââ¬Å"luxurious and imported itemâ⬠. She did not become ââ¬Å"famousâ⬠because she was hit in an accident, but the fact is because the ââ¬Å"Mercedes Benzâ⬠was involved in it. People are always curious about the rich and the famous. They are not interested in the ââ¬Å"insignificantâ⬠lower class people that are considered a burden to the Brazilian society. According to a journal article, ââ¬Å"For upper-class women, the result is a life of almost total leisure, in which they have servants to do all the work. For lower-class women, their survival often depends upon their ability to obtain one of these jobs. Approximately one-third of the women employed in Brazil work as household workers, including cooks, housekeepers, and child care workersâ⬠. (Brazil, 1987) Most of lower class women in Brazil work as housekeepers and child care workers to middle and high class families. Due to low or no education, they are only able to afford low skilled jobs. Yet, about two years ago, Brazilian television program has launched a reality show named ââ¬Å"Mulheres Ricasâ⬠, which means rich women. The reality show is about five very wealthy women who travel by private jet, eat from gold plates and women who would spend many thousands of dollars in clothing and imported bags. The show has been criticized internationally by comparing them to 11. 5 million Brazilian living in slums. The reason of the criticism is because it somehow proves hat the higher class only care about their own comfort by ignoring the real issues that is occurring in the country, which is the severe poverty. In Brazil, there is definitely a very big gap between the poor and the rich. Although the poverty is a big issue in the country, according to a recent report, Brazil has been creating 19 millionaires a day since 2007. Yet, the poverty in Brazil is causing several other problems such as th e drug traffic. Most of the times, the people who are involved in drug traffics, are the lower classes people. We can find people from all ages involved in the traffic. According to SOS Childrenââ¬â¢s village, ââ¬Å"In the countryââ¬â¢s largest cities, particularly in Rio de Janeiro and Sao Paulo, children without parental care often end up on the streets where they are vulnerable to gang violence, sexual abuse and drug addiction. ââ¬Å"(1) Rio de Janeiro, the second largest city in Brazil, has a place called ââ¬Å"favelaâ⬠where the drug traffic has been a problem for several years. The movie, ââ¬Å"Elite Squad: The Enemy Withinâ⬠by Jose Padilha, illustrates a ââ¬Å"fiction-realityâ⬠that occurs in Rio de Janeiro. The movie unquestionably shows to the viewers that ââ¬Å"human rightsâ⬠do not exist in Brazil, since the higher class people are the ones that hold all the power while the poor are not even recognized by the society. In the film, it mentions that the police would invade the ââ¬Å"favelasâ⬠killing all the poor people involved in the drug traffic; however, they would not invade luxury apartments killing or arresting people because they receive some commission from the rich. In other words, the police officers are tipped with the money from the drug traffic from ââ¬Å"higher classâ⬠people. Most of the times, if a person from a higher class commit a crime; they would use their money to avoid getting arrested and going jail. According to The Guardian, the Brazilian jail's population has doubled since the year 2000 due to the drug traffic. Of course, the majority of people who are in jail are from the lower class. Additionally, there is a scene in the film that might have caught many viewers attention. It is when the military killed one of the members in jail. The scene somehow send us a message by saying that it was not a big deal killing one of them since they are ââ¬Å"troubleâ⬠anyways. I believe that the movie certainly try to send a message by saying that Brazil needs to start listening to the lower class opinion and do not ignore or kill them without any reasons. The lower class and higher class people in Brazil share something similar and that is: both are humans. ââ¬Å"Human rightsâ⬠should be more clearly addressed in the country and the ââ¬Å"Rich rightsâ⬠should be banned. Most of Brazilian literary narratives and movies that we have watched in class, it involves children and adolescents working and not receiving any education. According to SOS Childrenââ¬â¢s Village, ââ¬Å"Quite frequently, young children have to engage in labour activities in order to put food on the table for an entire family. In the state of Piaui, approximately 26 per cent of children between 10 and 15 years of age are working. â⬠(1) Looking at the Brazilian demographic, Brazil is certainly a very young country with 25 percent of the population being under 15 year old. The country depends on these young populations for the economic and social growth in the future. Yet, looking at the national estimates, about 24,000 children and teenagers call the streets their home. The lower class children definitely have the disadvantage of not getting any education and no hope for a better future. While the higher classes children will still be the ones to succeed in life and dominate the country. If the inequality between the rich and the poor in Brazil does not change, the poor will always remain poor and it is possible that the problem can become more severe in the future. Even though many literary stories and movies composed by Brazilian authors are ââ¬Å"just storiesâ⬠or ââ¬Å"fantasiesâ⬠, some of them still try to send an indirect message to the public that ââ¬Å"social classâ⬠in Brazil is a problem and it needs to be changed. In addition, the movie, ââ¬Å"Four days in Septemberâ⬠, directed by Bruno Barreto, is about a reality that happened in Brazil years ago, involving the MR8 group, who was fighting against the military dictatorship and fighting for human rights. The group was formed by some young members, who were very naive, did not have much experience in life and did not think about future consequences that they could face by being involvedâ⬠in this type of activity. Thankful to this group formation, Brazil has overcome the military dictatorship. Although, the country has overcome the dictatorship, human rights are still a problem. Showing that you have money is a way to receive respect from people. In the movie, there is a scene where one of the young members of MR8 goes to a bakery to buy bread; however, the baker underestimated him by asking if he had enough money to buy. The way you dress, the way you speak and the amount of money that you hold is the way that people are going to treat you. If you dress badly and is considered part of lower class, your opinion will not even be considered and there is almost zero respect towards you. Therefore, I believe that many people in Brazil are confused between ââ¬Å"human rightsâ⬠and ââ¬Å"classâ⬠. According to the federal government in Brazil, ââ¬Å"The following human rights problems have been reported: unlawful killings, excessive force, beatings, abuse, and torture of detainees. â⬠(1) Most of the times, the people who are suffering under those problems are the innocent lower class population, who is sometimes always blamed for any issue that occur in the country. In, it is correct to assume that ââ¬Å"moneyâ⬠can solve anything. On the other hand, since the lower classes do not have enough money, they get severely punished for their actions or even blamed for things that they have not committed. All in all, even the literary stories and movies are fictional, the Brazilian authors or directors seem to always try to incorporate the fiction with the reality that is faced in Brazilian society. As we could see from the short stories and movies, ââ¬Å"classâ⬠is a very big issue and it is confused with ââ¬Å"human rightsâ⬠. The poor still face several consequences while the higher classes ignore the surrounding by using ââ¬Å"moneyâ⬠to solve most of their problems. Inequality between the lower class and higher class is a big issue in Brazil that has to be solved before getting worse. It is also crucial for the country to understand the difference between ââ¬Å"human rightsâ⬠and ââ¬Å"classâ⬠. I believe that if the poor is listened and given more opportunities in the society, they will also be able to succeed in life. It will not only decrease the poverty in the country, but it will prove to the people that the people from the lower classes also have the potential, but the only thing missing is the opportunity given to them.

American Music and Culture: Jazz Dance Jazz dance, is defined as any dance to Jazz accompaniments, composed of a profusion of forms. Jazz dance paralleled the birth and spread of Jazz itself from roots in black American society and was popularized in ballrooms by the big bands of the swing era (1930s and ââ¬â¢40s). It radically altered the style of American and European stage and social dance in the 20th century. On the stage, minstrel show performers in the 19th century developed tap dancing from a combination of Irish Jigging, English clog dancing, and African rhythmic stamping. Tap dance and such social dances as the cakewalk and shuffle became popular vaudeville acts and appeared in Broadway revues and musical comedies as these replaced vaudeville early in the 20th century. In addition, comedy, specialty, and character dances to Jazz rhythms became standard stage routines. By the 1940s elements of Jazz dance had appeared in modern dance and in motion picture choreography. The History of Jazz Dance During the early 1900ââ¬â¢s we find more and more blacks performing outside of the narrow stereotype of the minstrel show. Again the traveling shows spread the music and dance culture of black people far and wide. In addition to the spreading of culture, there was another important aspect to the events taking place: it was the beginning of the musical theatre. The black musical revue offered comedians, singers and dancers an opportunity to perform without making fun of their race. Out of such erformances, one became aware of new dance developments: cakewalks, grinds, comedy dance, etc. Another important factor in this discussion on the historical development of modern Jaz dance is always the constant dance involvement and development in the everyday lives of black people. Dance has always been a part of the expression of black people in church, at social gatherings, etc. Probably the social or vernacular dance, as Stearns calls it, is more important than any one particular form that evolved out of it. So as the dance of the musical revue evolved, it was directly associated with and tied to the everyday dance of the people. Prior to 1900 there were such black shows as The South Before the War, The Creole Show, Oriental America, etc. Many of the dance movements associated with Jazz dance can be traced to Atrican intluences. Slaves captured in Atrica brought their dancing traditions across the Atlantic. Once in the new country, the African slaves continued to use dance as a means of self-expression and an emotional outlet, despite being forbidden to dance by their owners. Until the mid 1950s, the term ââ¬Å"Jazz danceâ⬠often referred to tap dance, because tap dancing (set to Jazz music) was the main erformance dance of the era. During the later Jazz age, popular forms of Jazz dance were the Cakewalk, Black Bottom, Charleston, Jitterbug, Boogie Woogie, Swing dancing and the related Lindy Hop. Although the stage popularized certain social dances, many others were transmitted mainly in social gatherings. The dances that gave rise to social forms of Jazz dance developed from rural slave dances. In both early dances and 20th-century Jazz dances, there is a noticeable continuity of dance elements and motions. The eagle rock and the slow drag (late 19th century) as well as he Charleston and the Jitterbug have elements in common with certain Caribbean and African dances. In addition, the slow drag contributed to the fish of the 1950s; the ring shout, which survived from the 18th into the 20th century. After the 1950s, pioneers such as Katherine Dunham took the essence of Caribbean traditional dance and made it into a performing art. With the growing domination of other forms of entertainment music, Jazz dance evolved on Broadway into the new, smooth style that is taught today and known as Modern Jazz, while tap dance branched off to follow its own, separate evolutionary path. The performance style of Jazz dance was popularized to a large extent by Bob Fosses work, which is exemplified by Broadway shows such as Chicago, Cabaret, Damn Yankees, and The Pajama Game. Modern Jazz dance continues to be an essential element of musical theatre, and it can often be seen in music videos and competitive dance. Jazz Dance Techniques Throughout its history, Jazz dance has developed in parallel to popular music. This pattern of development has resulted in a few elements of movement key to the dance style, the most important being that Jazz is they physical embodiment of the popular music of a given time. Jazz Dance Pioneers and African Americans Katherine Dunham was an anthropologist fascinated by dances of the Caribbean. She researched them and then devised her own style of dance and teaching methods. She went on to dance on Broadway and to form her own school. Her school in New York was very influential in the 1950s and her technique of pelvic and spine isolations is now a part of almost every Jazz class. Jerome Robbins was the greatest Broadway Jazz choreographer of our time. He dealt with action, humor and adolescent themes in his Broadway musicals and films. He choreographed Fancy Free and Westside Story among others. His choreography was powerful and appealed to huge audiences. His influence on Broadway is still felt today. Gus Giordano has received many awards for his outstanding lifetime contribution to Jazz dance. He has been involved in all facets of dance: dancer, teacher, choreographer, author, and founder of Gus Giordano Jazz Dance Center in Chicago. Giordanoââ¬â¢s book, Anthology of American Jazz Dance , was the first book of its kind and was instrumental in helping turn the study of Jazz dance into a respectable and important American dance form. Vaudeville, or the variety show, was initially a European tradition of traveling performers moving from town to town with their skits, songs and dances.
